- Apparently it is downloaded and stored on your iPhone, but it's also backed up at the company server for resynching purposes. Thus the amount and time limits.

About the visual voicemail, I am already subscribed. I just haven't gotten any calls yet.
In addition to the 315 yen/month service charge there is also a charge for the time you listen to the messages, just as though you had made a phone call, right?
